Requests
the necessary information to register for the Crew Program. Click

Athletic Participation/Parental
Consent/Physical Exam Form
Required
by Fairfax County School System, this form must be completed fully
and signed by the Doctor’s office and the parent by the registration
dates if any student is to participate in any athletic program.
The actual physical examination cited by the participant’s physician
must have been completed after 5/01/07. Click

Office.
No one may participate in Crew winter-conditioning or practice without
a completed form being turned in.
Emergency Care
Information Card
Required
by Fairfax County School System, this form must be completed by
the start of winter-conditioning (12 Nov). Click

Office.
Langley
Crew Rules and Guidelines
Parent
and student are required to sign the last page of the guidelines
acknowledging they have read and understand the rules and guidelines.

County Public Schools Participation Policy
Extracurricular Activities and Athletic Programs
Required
by Langley High School, this handbook must be read and the last
page signed by both the student and, if the student is under 18,
by a parent, if the student is to participate in any athletic program.

Swimming
Requirement Form
This
requirement pertains only to new team members. Each participant
must pass the Swimming Requirement set forth by the National Capital
Area Scholastic Rowing Associations (NCASRA) before joining Crew.
Swim Tests will be held in February (date TBD) at the Spring Hill
Recreation Center. Click for more info. (Will be posted Jan '08)
U.S.
Rowing Insurance Waiver Form
This
waiver form indicates that the student and his/her parents understand
rowing activities and believe the student is qualified to participate
in this program. It indemnifies, saves, and holds harmless each
of the Releasees from any claim by the participant. No one may participate
in any Crew practice or regatta without a completed form on file.

Field
Trip Driver’s License and Vehicle Insurance Information
Required
by Fairfax County School System for any student or parent who is
involved in transporting participants to and from practices and

to ride with someone other than themselves to and from practices
